Insurance Information

Middle Path Counseling is currently able to accept Aetna, Cigna, and Select Health insurance as an in-network provider.

Contract applications are also in progress with UnitedHealthcare and Kaiser.

Using Insurance

If you have Aetna, Cigna, or Select Health insurance, I can file claims with your insurance company on your behalf as an in-network provider. Your insurance plan will determine the amount covered and any portion that is your responsibility.

Because coverage varies significantly between insurance plans, please check your individual policy for specific details about mental health or outpatient behavioral health visits, including:

  • Copays or coinsurance

  • Deductible requirements

  • Visit or benefit limitations

  • Any other applicable plan requirements

As a general estimate, client responsibility is often around $15–$30 per session in the form of a copay or coinsurance. However, this varies widely by plan, and your actual responsibility may be higher or lower.

Your Responsibility for Payment

Although claims are filed with your insurance company on your behalf, you are ultimately responsible for any amount your insurance plan determines that you owe. This may include copays, coinsurance, deductibles, or other charges not covered by your plan.

Payment of any client responsibility, including copays, coinsurance, deductibles, or self-pay fees, is due at the time of the scheduled session.

Insurance benefits can vary and are not guaranteed until a claim is processed. You are responsible for understanding your insurance coverage and confirming your benefits directly with your insurance company.

For the most accurate information about your coverage, contact the member services number on the back of your insurance card and ask about your benefits for outpatient mental health services.
